If you have received a Qualification Questionnaire from one of the New York State Unified Courts, then, what should you do? You must to complete this questionnaire. We encourage you to complete and submit your Questionnaire online by using the Juror Online Qualification system at www.nyjuror.gov/qualify. This can help you to save a stamp, save your time and bring about conveniences. To complete your Questionnaire online, you should first access the Juror Online Qualification system and click on the link at the bottom of the home page. Then you will be prompted to enter your Juror Index Number and answer some questions about your personal information.

If you decide to complete your paper Juror Qualification Questionnaire, you should tear-off, fill it out, sign, and return it to the court within 10 days.

The main aim of this Qualification Questionnaire is to determine who is qualified to serve as a juror. It's not a subpoena. Qualified jurors will be called for federal jury service within the next two years.
