If you have received a letter from Aon Hewitt that ask you to provide copies of documentation that substantiates the relationship to you of any dependent who is covered by the health, dental, vision, and/or tuition discount benefit, there are three convenient ways for you to submit your documentation to Aon Hewitt: by secure fax, by mail and by online upload at www.yourdependentverification.com/plan-smart-info.

To log in to www.yourdependentverification.com/plan-smart-info, you have to enter the Login Name and Password. The default Login Name is "PS + Your Dependent Verification ID" and the default Password is your date of birth in mmddyy format.
